As part of providing a counselling service, Balanced Living Counselling & Consulting is committed to providing you with a quality service which includes ensuring that your privacy is maintained.

I am required to collect and record personal information from you that is relevant to your current situation.

 

This information will be a necessary part of the counselling that is conducted. The information is retained to document what happens during sessions and enables the Counsellor to provide a relevant and informed counselling service.

All personal information gathered by the Counsellor during the provision of the session will remain confidential and kept in a secure location except where:

It is subpoenaed by a court, or

1.  Failure to disclose the information would place you or another person at risk; or

2.  Provide relevant information to another professional, such as or if disclosure is otherwise required or authorised by law.

3.  Your prior approval has been obtained to:
    a) provide a written report to another professional or agency
    b) discuss the material with another person (e.g., a parent)
